1. Providing Personal data
Personal data which I give consent to the Company to compile, use or disclose
General personal data: name, family name, photo, gender, date of birth, age, nationality, ID card, passport, expiry date of ID card and passport, passport issuing country, occupation and other relevant information (in case of application for VISA);
Information on Payment: information of credit card payment, bank transferred document, ibanking document, pay-in slip, details of the bank, currency;
Information on Travel: date and time of travel, the source country and the destination country, tourist’ attractions, transportation, food or goods and service which the applicant intends to buy and use;
Contact Information: phone number, mobile number, e-mail, addresses;
Information on Health: chronic disease, pregnancy, medical certificate, vaccination, treatment record;
Information on Insurance: travel insurance;
Other information: any other information which the applicant provides to the Company.
This shall include providing information in verbal or original or a copy of the document.
2. Objectives of the Compilation, Usage or Disclosure of Personal Data
2.1 To carry out to apply for Thailand Elite visa and membership and transfer an applicant’s personal data to Thailand Privilege Card Co., Ltd. to proceed the visa application.
2.2 To disclose the personal data under Clause 3. hereof.
3. Disclosure of the Personal Data
For the purpose of service of Thailand Elite visa application and any action in accordance with Clause 2 hereof, the Company shall disclose the personal data to person and/or the agencies as follows;
3.1 Thailand Privilege Card Co., Ltd.;
3.2 staff of Company, Company’s affiliated companies or its branch offices;
3.3 credit information checking-service provider or marketing advisor;
3.4 legal advisors, lawyers;
3.5 state agencies, courts, police, customs, immigration office, airports, embassies, security agencies or when the Company received order of the state officials.
4. Storage and Compilation of the Personal Data
The Company shall store and compile the personal data of applicants. In this connection, the Company imposes the appropriate measure to store and compile the data and such measure may be adjusted to be in line with the relevant laws as follows:
4.1 To determine the staff having duty to store and compile the personal data;
4.2 To control and to restrict the right to access the data as appropriate.
5. Period of Time of the Compilation, Usage or Disclosure of Personal Data
The Company shall compile, use or disclose the personal data of applicants, starting from the date on which the applicant signs this Consent Letter until the applicant’s Thailand Elite card has been issued, and Thailand Elite visa has already been affixed, or according to the period of time which the Company deems that it is necessary for taking action according to the objectives or provision of laws.
6. Source of Personal Data
The applicant affirms and confirms that he/she is the owner of the personal data and has right to give consent to the Company to compile, use or disclose the personal data according to this Consent Letter and provision of laws;
In case that the applicant is not the owner of the personal data, the applicant affirms and confirms that he/she is authorized and obtains consent according to laws from the owner of the personal data to be agent in giving consent according to laws;
In case that the owner of the personal data is a minor, incompetent person or quasi- incompetent person, the consent according to this Consent Letter shall be given by the lawful representative, guardian the representative stipulated by law.
7. Rights of the Owner of the Personal Data
7.1 To access or to copy his/her personal data which is under responsibility of the Company;
7.2 To ask for disclosing the personal data which he/she has not given consent;
7.3 To ask for receiving the personal data relating to him/her (in case that the Company organizes the data to be in form that it can be read or used in general by tool or equipment which can work automatically, and such data can be used or disclosed by automatic method);
7.4 To send or transfer his/her personal data in form that it can be read or used in general by tool or equipment which can work automatically, to other person;
7.5 To object the storage, compilation, usage or disclosure of his/her personal data anytime;
7.6 To ask for erasing or destroying personal data or to make such data to be unable to identify its owner;
7.7 To ask for suspending the usage of his/her personal data;
7.8 To ask the Company to correct or to make his/her personal data to be accurate, updated, complete and such data shall not result in any misunderstanding;
7.9 To ask for revocation of consent according to this Consent Letter anytime according to the condition stipulated in Clause 9. hereof;
7.10 To complain in case that the Company or employees or the processor does not comply with the personal data protection act. In exercising the right of personal data owner, it shall comply with the provisions of law in all respects.
8. Revocation of Consent
The applicant may revoke his/her consent of the storage, compilation, usage or disclosure of his/her personal data according to this Consent Letter anytime. The applicant shall notify Company for acknowledgement by sending such written notice via mail or electronic mail.
If the applicant revokes his/her consent of the storage, compilation, usage or disclosure of his/her personal data, such revocation may affect his/her Thailand Elite visa application.

In giving the consent according to this Consent Letter, I have thoroughly read and well understood the contents in this Consent Letter and I consent to the Company to compile, use or disclose the personal data subject to the above-mentioned terms and conditions.
